On-Line Recovery Form – First Responder In Charge

This form is for the IUCRR team member in charge of the rescue/recovery (Surface Operations Manager). It is critically important that you provide detailed info, as quickly as you can. If, later, you need to amend your initial or subsequent reports, that is not only acceptable but perfectly fine. But the initial report should be submitted within 3-5 days of the recovery.

Please complete all fields that you can. It’s better to have too much information, than not enough.

If you do not click the ‘Submit’ button at the bottom, after you’ve entered the information, then it will NOT be sent to the IUCRR.
